The Greater Lowell Community Foundation annually honors two individual volunteers and a youth community service club for their exemplary efforts of improving the quality of life in our service area. Awards will be presented at "Celebrate Giving!" the region's most prominent event of philanthropy\, on November 8 2012. Take just a couple of minutes to nominate a deserving individual or group and you can change their lives as they are changing the lives of others.\n\nLifetime Achievement Volunteer\n\nAn adult who has demonstrated a long-term commitment to community service in the Community Foundation's service area\, achieving measurable results towards fulfilling community needs.\n\nPrize: $1\,000 to the honoree's choice of charitable organizations.\n\nBruce C. Leggat Rising Star Volunteer\n\nA youth (21 years or younger) who has embraced community service\, inspired others in his or her work\, and is on track for continued contributions to his or her community.\n\nPrize: $1\,000 to the honoree's choice of charitable organizations.\n\nYouth Community Service Club\n\nA school or community-based service club that has successfully engaged numbers of youth in service\, and has made a real impact towards improving life in the Community Foundation's service area.\n\nPrize: $500 to the winning club.\n\n \n\nHOW TO SUBMIT YOUR NOMINATION\n\nPlease choose one of the three award categories\, and complete the appropriate attached form. The deadline for submission is October 25\, 2012. Or download a nomination form online at www.glcfoundation.org. The form for the Lifetime Achievement Winner and Rising Star is the same\; simply check the appropriate award at the top of the form. Submissions may not exceed two pages. However\, up to five flyers\, photos\, press materials\, and other supporting documentation may be submitted as supplements. Provide as much information as possible about the volunteer or service club's activities and how they have positively influenced the community.
